Laura Dwelley, BSN, RN, CDP
Laura Dwelley is a specially trained Registered Nurse and Certified Dementia Practitioner. Laura first began her career in healthcare as a volunteer for a local hospital in the late 80’s. Now, self-employed as a concierge RN Care Manager and Healthy Lifestyles Coach, Laura brings her life-long desire to help others to a practice where she specializes in making a difference for her clients.
Laura loves to learn and grow her knowledge of health and wellness. Currently studying natural healing and functional medicine, Laura holds a Bachelor of Science Degree in Nursing from the University of Phoenix, an Associate of Science Degree in Nursing from Goodwin College, and an Associate of Science Degree in Business Administration, from the New England College of Business & Finance.
Laura is passionate about patient advocacy, nutrition, lifelong learning, and supporting her clients with compassion and empathy. Laura’s background includes emergency medicine, serving 7 years on a volunteer ambulance crew and member of the board, pediatric care, and geriatric home health care. Over the past decade, Laura has concentrated her education and career on geriatric care, working in case management, Alzheimer’s adult day centers, and care management.
Originally from New England, Laura and her husband have called Florida home for the last 12 years. In her free time, Laura enjoys spending time with her family, gardening, and creating art through up-cycling architectural salvage and unique Flea Market finds.
Christine Conti, DNP, MSN, RN, CDP
Dr. Christine Conti is a compassionate care partner with more than fifteen years of nursing experience as well as advanced doctoral training. Her clinical background includes years of hands-on practice as a staff nurse, where she developed strong skills in holistic assessment, care coordination, and advocacy for the geriatric population. As a Certified Dementia Practitioner and Care Manager, Christine brings a gentle, patient-centered approach to supporting older adults living with cognitive changes, chronic conditions, and complex health care needs.
Christine is dedicated to helping older adults feel supported, understood and empowered. She values collaboration with families and interdisciplinary teams to ensure that each client’s plan of care reflects their individual goals, strengths, and unique needs. Christine is deeply committed to this work and finds great purpose in supporting this population, describing her time with clients as both meaningful and profoundly rewarding.
In her spare time, Christine enjoys hiking, fishing, and boating with her husband as they pursue their shared bucket-list goal of visiting every U.S. national park. She also values time with family and friends and finds great joy in serving underserved communities in the United States and abroad.
Chelsea Culberson, CMA
As a mom, a certified home health aide, and a former business manager, I bring a blend of real-life experience and practical skills to every family I support. Caring for seniors is truly meaningful to me and I approach each person with patience, kindness, and a gentle touch. My goal is to offer compassionate, non-clinical care for your loved ones, so you can feel confident knowing they’re in good hands. I also assist with dependable logistical and technical support for LM Services, LLC.

Dianne Layden, RN
Dianne Layden is a Registered Nurse and Certified Nurse Care Manager based in Stuart, Florida. With a Bachelor of Science Nursing degree from New York University and is an Advanced Member of the Aging Life Care Association.
Dianne has garnered over 30 years of experience in hospital settings, home health care, insurance industry, and care management.
Dianne's expertise lies in comprehensive assessment and care coordination, managing complex care for seniors with multiple chronic conditions. Dianne is committed to ensuring patient safety and delivering the highest quality of care to her patients. She is also highly skilled in communicating with clients, family members, doctors, and other professionals.

Lauren Ragonese, RN
Lauren has been a nurse since 2001, originally from Connecticut. While in Connecticut, she worked in the hospital setting as well as home health- including 2 years as a psychiatric home health nurse.
In 2011, Lauren moved to Jupiter, FL, where she worked as an RN Case Manager for a Medicare home health and private duty company. She later transitioned into senior living, serving in various Director roles across independent living, assisted living, and memory care. In 2020, Lauren returned to private duty home health and currently works as a Registered Nurse at Visiting Angels, Palm Beach.
Lauren is deeply committed to working with seniors, particularly those living with dementia. She finds fulfillment in supporting individuals through the aging process, including providing compassionate care during end-of-life transitions.
In her personal time, Lauren is very involved with her two daughters and enjoys spending quality time with her family. Lauren also enjoys walks on the beach, working on small projects that bring out her artistic ability, and reading motivational content.
Some of Lauren's greatest qualities are being very detail-oriented, honest, personable, and having the ability to make people feel comfortable and supported.
